Family Care focuses on the achievement of positive outcomes for its patients thereby allowing patients to remain in their homes. Outcomes that are targeted include improvement in activities of daily living, mobility, disease management, and prevention of hospitalization. Patients and family members are encouraged to participate in their care so that they may speed the recovery process and enjoy the highest quality of life.
