Family Care provides a full range of professional health care services, 24 hours a day, seven days a week to clients throughout Richmond and its contiguous counties as well as the Tri-city area including Petersburg, Hopewell, Colonial Heights, Dinwiddie, Prince George, Sussex, and portions of Surry counties.

Skilled Nursing Services includes:

  • Wound care
  • Medication management
  • IV administration and infusion therapy
  • Foley catheter changes
  • Cardiac and diabetic assessments and instructions
  • Disease management
  • Pain management
  • Post operative care

Therapy Services provided by Physical, Occupational, and Speech therapists include:

  • Home safety evaluations and recommendations.
  • Evaluation and assistance with ordering adaptive/assistive devices to promote independence and a safe, maximal level of functioning.
  • Evaluation and treatment interventions to improve mobility, walking, balance, strength/endurance, and joint function.
  • Evaluation and treatment interventions to improve the ability to perform self-care skills and activities of daily living (such as grooming, bathing, and dressing).
  • Development of individualized exercise and treatment programs.
  • Evaluation and treatment of communication and swallowing disorders.
  • Pre-operative home visits are provided as an option to any patient scheduled for joint replacement surgery.

Personal Care Services provided by certified nursing assistants or companions include the following services:

  • Hourly or live-in care
  • Bathing, dressing, and other activities of daily living
  • Meal preparation
  • Light housekeeping
  • Shopping for groceries
  • Transportation to doctor visits and activities
  • Companionship
  • Medication prompting

Social Work Services include:

  • Assistance with social and emotional problems
  • Long term care planning
  • Accessing reimbursement for services
  • Accessing community resources

Our Visitation Program consists of visits to private homes.  This service can provide distant relatives reassurance that their loved ones are receiving appropriate care, and that their needs are being met in their current environment. Visits will be made by nurses, social workers, or whatever appropriate level of staff is needed to assess a client’s needs and care. Visitation staff can assess the following:

  • The safety of elderly individuals living alone
  • Medication management
  • Living conditions
  • Health status
  • Adequacy of resources

Visitation staff will contact the legally responsible party with pertinent information and submit regular reports regarding findings. They may also assist in setting up a new plan should one be needed. Utilize this service to give you the peace of mind you seek.

We coordinate our services with all referral sources and agencies as well as family members.
